Work Permit Information

Springfield Prep can ONLY issue permits for SPCS students ages 14+.

The following sections of the Employment Permit Application Packet must be completed before submitting to SPCS:

• Promise of Employment completed and signed by Employer.

• Certificate of Health signed by your child’s physician.

• Parent/guardian signature completed by child’s parent or legal guardian.

• Signature of Minor completed by student.

Completed applications, along with a copy of your child’s birth certificate, can be emailed to Ashley Major at amajor@springfieldprep.org or brought to the school’s Main Office.

SPCS will review the submitted documents and complete a School Record form for each applicant.

Once all documentation is complete, SPCS will issue a Youth Work Permit for the student. This document must be returned to the student’s employer, and a hard copy will be kept in the student’s file at SPCS.
